Initial DisclosureA breach notice posted by the Office of the Maine Attorney General says TriZetto Provider Solutions affected over 3.4 million individuals, and TPS says compromised data may include names, addresses, dates of birth, Social Security numbers, health insurance member numbers including Medicare identifiers, provider names, health insurer names, and other demographic, health, and health insurance information.
